“So how bad was it?” This is typically the first question that gets asked when we start dealing with a patient safety event or error. The answer to that question guides our future approach as we move to actions that improve our ability to provide safe care. While historically, there has been hesitancy to expand our analysis beyond the direct “harm” impact on the patient, we need to expand our view to look at Total Impact to ensure that our patient safety program is providing the maximum benefit to the organization.
