| | This ebulletin contains a selection of HSE's latest prosecution notices: Firm fined more than £500,000 after worker falls to their death The worker was attempting to clear a machinery blockage when he lost his footing, falling to his death. £225,000 fine issued after worker dies from fall through roof The fine was issued after an investigation found the work at height was not properly planned, resulting in a worker fatally slipping and falling through a fragile roof. Company fined after worker crushed to death The £300,000 fine was issued after a worker was crushed by 2 stacks of concrete blocks at a construction site. Farmer fined following death of worker The worker was killed when the overloaded quad bike they were using for farm work overturned, causing fatal spine injuries. Manufacturer fined £100,000 after worker injures hand The worker was cutting a length of windowsill when their hand slipped into an unguarded blade, severing parts of 2 fingers. Business owner jailed over deaths of 4 people The paddleboard tour leader has been jailed for 10 years for manslaughter after a trip in a fast-running river resulted in the deaths of 4 people. | No Falls Week 2025: 12 to 16 May HSE's statistics show that falls from height were the main cause of worker fatalities in in 2023 to 2024, with 50 workers killed.
