  When investigators visited the VA nursing home in Brockton, Massachusetts, to look for potential signs of patient neglect, they didn’t have to look far: They found a nurse and a nurse’s aide fast asleep during their shifts. One dozed in a darkened room, the other was wrapped in a blanket in the locked cafeteria, according to a report by the investigators who said they had “significant concern about the blatant disregard for veteran safety.” The Brockton nursing home is one of 45 to receive the lowest one-star rating in new VA quality rankings, released after a joint investigation by USA TODAY and The Boston Globe.
