In order to provide transparency into state directed payments, the Centers for Medicare & Medicaid Services (CMS) is publishing all state directed payment preprints on Medicaid.gov on the following page:Approved State Directed Payment Preprints. All state directed payment preprints approved on or after February 1, 2023 will be posted, and the site will be updated regularly.

We would also encourage states to use our revised508-compliant state directed payments preprint formavailable on our guidance page, as well as thePreprint addendum tablesin an Excel workbook. The Excel workbook addendum to the state directed payments preprint form allows states more rows than is provided by the eight tables in the preprint. The addendum tables are organized by tab. Within the tables, states will find data elements with specific drop-downs that CMS has pre-selected to standardize data provided by states in the preprint.

CMS requests that states submit the state directed payment preprint for CMS review in the fillable PDF format; please do not merge the form with additional files or re-PDF the document. We are also asking states to use the addendum tables rather than submitting separate documents (Excel files, etc.) to respond to questions in the preprint form.
